I understand that by joining the CSA, I pledge my financial support in exchange for Heartbeat's best effort to provide me with a monthly care package of herbal splendor for the duration of my membership.

I understand that I must pick up my monthly share at the farm, which is 3 1/2 miles north of Yellow Springs, Ohio, during the specified pickup hours.

I understand that if I am unable to pick up my share during the designated pickup time, it is my responsibility to make alternative arrangements.

I understand that Heartbeat prepares each month's shares based on the number of members and therefore does not guarantee refunds in the event I decide to cancel my CSH membership.

I understand that the herbshares are offered to support my home healthcare practices, and do not take the place of seeing my licensed healthcare provider. The herbshare program is not intended to diagnose, prescribe or treat illness. I understand I need to consult my health care provider before using herbal supplements.
