Online Sign Up

The CSA begins in June. You may pay in full when you sign up, or choose a 3 payment installment plan. If you choose the latter option and make your first payment, we will send you invoices for June 1 and August 1. If shares are still available after June 1, contact us about a prorated price. To sign up, fill out the information form and make a payment. You may do this online using PayPal, or by mail with a printed form and check. Your share cannot be reserved until payment is received.

I understand that by joining the CSA, I pledge my financial support in exchange for Heartbeat's best effort to provide me with a weekly supply of fresh produce from June to October.

I understand that I must pick up my weekly share at the farm, which is 3 1/2 miles north of Yellow Springs, Ohio, during the specified Thursday pickup hours.

I understand that if I am unable to pick up my share and have not made arrangements to pick it up at a later time, the food will be donated to a local soup kitchen or used for the farm training program.

I understand that Heartbeat does not guarantee refunds in the event I decide to cancel my CSA membership.

Large Share, Full Payment ($500)

Medium Share, Full Payment ($400)

Medium Share, 3 Installments ($140) – we will send invoices June 1 and August 1

Small Share, Full Payment ($285)

Small Share, 3 Installments ($100) – we will send invoices June 1 and August 1
